If so, an exciting opportunity has arisen for an experienced HCA in general practice to join our multidisciplinary team working across the Glossopdale Community. Main duties of the job The post holder will be a skilled and experienced Healthcare Assistant responsible for supporting the care of patients with a variety of acute and long-term health conditions. As part of the new Team Up initiative, they will provide care within their professional scope through community-based home visits. Their role will include delivering and assisting clinical staff with treatment, preventative care, health promotion, and patient education. Key responsibilities include performing baseline clinical observations such as pulse oximetry, blood pressure, phlebotomy, BMI, and temperature checks as directed, and accurately recording results. The post holder will also contribute to chronic disease management and deliver opportunistic health promotion where appropriate. They may be involved in completing basic care plans. About us Glossop Primary Care Network (PCN)serves a population of approximately 33,000 patients across four practices: Manor House Surgery, Cottage Lane Surgery, Lambgates Health Centre, and Simmondley Medical Practice. Competency in Phlebotomy. Full UK driving licence and access to a vehicle. NVQ Level 2 in Health & Social Care or equivalent experience.
