The H&N Clinical Nurse Specialist role will incorporate the dimensions of expert clinical practice, education, leadership, management and research.
The post holder will be required:
· To liaise with all key members of the multidisciplinary team to advise and support colleagues in the delivery of specialised patient care across professional boundaries.
· To work autonomously to deliver specialist clinical care appropriate to the needs of the patient group and based on principles that are research based and enhance the quality of patient care.
· To undertake nurse led clinics and to be responsible for excellent standards of nursing care to patients through the planning, co-ordinating, delivering and evaluating nurse-led clinics and services.
· To lead and support with specialist nursing knowledge the development, and implementation of programmes of nursing care for individual patients and the client group as a whole.
· To contribute to the facilitation of the planning, delivery and evaluation of cancer pathways for a designated group of patients.
· To contribute to training and education within specialist area.
· To promote an innovative and progressive attitude to the continual improvement of patient care through reviewing the evidence base and embedding this in practice through engagement in research activities.
· To act as a visible role model and expert practitioner.
Main duties of the job
To lead on the provision of a clear focus for nursing clinical leadership across the H&N nursing service and to be responsible for ensuring the delivery of high quality patient care
To be responsible and accountable for the provision of expert and specialised cancer care, in collaboration with the H&N cancer MDT
To represent the Trust and the speciality at internal and external meetings related to the H&N service.
To maintain a patient-centred approach when planning care, involving patients, carers and members of the MDT in care decisions
Ensure quality is central to clinical practice by delivering evidenced based individualised care, through holistic needs assessment, planning and evaluation of care promoting personalised at all times
Support patients in the delivery of care and meeting their health and wellbeing needs by providing expert advice and information, promoting their wishes and beliefs and addressing their concerns

Detailed job description and main responsibilities
MAIN DUTIES
-The post holder is responsible for the delivery of safe and effective specialist
nursing care of patients with head and neck cancers throughout their cancer
journey. This includes recognising and assessing complex needs which may require
specialist intervention.
- They will assess, plan, implement and evaluate individualised programmes of
evidence-based nursing care for the head and neck cancer patient and their
families utilising agreed assessment tools in collaboration with other key
professionals.
- The role requires the nurse to support medical staff in the out-patient department,
hospital wards and primary care settings, and act as patient advocate to ensure
the delivery of care is focused on the head and neck cancer patient and family.
-The post holder will support the close monitoring of cancer patients to meet
waiting time standards.
-They will be actively involved with MDT planning, implementation and outcomes.
-They must be aware of resources/ referral criteria which enhance the
patient/carer experiences.
- The post holder will act as team leader to other members of the Head and Neck
team.
- They will act as a Key Worker for a clinical caseload, working to coordinate
care and provide easy access for ongoing support for patients.
- The post holder will assess people’s information needs and provides relevant
information to meet those needs.
- They will enable people to access, identify, appraise, and interpret
information, to determine their own future and promote a culture that
supports self-management where appropriate.
- Provides clinical expertise, specialist advice and support across service
boundaries.

* Post-basic qualification in cancer/palliative care or relevant subject – or equivalent experience
* Evidence of continued professional development
