At Change Grow Live, you will work with people to help them change the direction of their lives, grow as individuals, and live life to its full potential.
Making the decision to enter treatment can be intimidating, so creating a welcoming and caring environment that is non-judgmental is essential. Your role involves listening to service users, understanding their needs, and supporting them to access the appropriate interventions at the right time. You will advise on available options within the service and the wider community, offering knowledge, advice, and equipment to help them stay safe. All necessary skills, training, and support will be provided.
Responsibilities
1. Support the team in completing triage assessments for new service users.
2. Provide information on treatment options available both face-to-face and via telephone.
3. Assist service users in accessing treatment and support services.
4. Ensure accurate and timely recording of client data in compliance with service requirements.
5. Conduct screening processes including urine, saliva tests, and Dry Blood Spot tests.
6. Offer advice and information to help service users maintain their safety and well-being.
7. Signpost and refer service users to community support resources.
