Overview
Are you an RMN or OT with community and dementia experience? Intensive Community Support Dementia team are looking for a Clinical Team Leader to join. We are a specialist service in which Community Mental Health Teams can refer to when a patient’s behaviours become more challenging. We offer high quality interventions to help patients in their own environment, which could be either a care/residential home or in their own private home.
Responsibilities
* As a Clinical Team Leader you will be dynamic and highly motivated, with exceptional assessment, risk management and care formulation skills.
* Demonstrate strong communication skills and clinical decision-making.
* Undertake shift management to ensure the smooth running of the team and use resources effectively.
* Cover North and West Dorset; role is community-based and a driver is essential (pool cars available).
* Work with CMHTs, social care and other agencies to benefit patients and support carers while patients are under our care and signpost for ongoing support.
* Undertake assessments, nurse reviews, manage a small group of patients and triage referrals as they come in.
* Contribute to and be responsible for implementing service changes as guided by the National Dementia Strategy and apply the Recovery Agenda to Organic services.
* Process information for ICSD as a “virtual ward” quickly and accurately.
* Complete a Prevention and Management of Violence and Aggression course; successful completion is a condition of employment.
Qualifications and Requirements
* Essential: MHN/OT qualification with community and dementia experience; driver (North and West Dorset coverage).
* Understanding of MHA, MCA and safeguarding legislation.
* Newly qualified nurses/occupational therapists/physiotherapists are welcome; a comprehensive Preceptorship Programme is provided.
* Eligibility to undertake the role’s required training and the aforementioned course before start date.
Service Details
ICSD is a 7-day-a-week service, operating from 07:30 to 20:00 on a rota basis (07:30–15:30 / 12:00–20:00) with alternate weekends at enhanced pay. This role is part time with a minimum of 30 hours per week, with a hybrid approach welcomed. Based at Forston Clinic, Dorchester with opportunities for remote working.
