What you will do: Manage a varied caseload of adults with a range of care and support needs within the community. Undertake assessments, care planning, and reviews under the Care Act 2014. Work collaboratively with health professionals, families, carers, and other agencies to develop person-centred care plans. Apply a strengths-based approach to practice, promoting independence and empowerment. Ensure safeguarding procedures are followed and contribute to safeguarding enquiries when necessary. Maintain accurate and timely case notes and records in accordance with local and national guidelines. Participate in multidisciplinary team meetings and support service users through care transitions. Requirements: Qualified Social Worker with registration with Social Work England. Sound knowledge of legislation relevant to adult social care, including the Care Act 2014, Mental Capacity Act, and Safeguarding Adults. Experience of working within a Locality or similar adults' team in a local authority setting. Strong communication, assessment, and interpersonal skills. Ability to work independently and manage workload efficiently. Enhanced DBS check required.
