You will treat patients with acute exacerbations of chronic diseases, manage long term conditions and support the assessment, plan, and treatment of patients to prevent them being admitted to hospital. You should have excellent clinical, written, and verbal communication skills, be able to work independently and manage your own workload. For purposes of the role, you must be NMC registered, hold a full UK driving licence, have good IT skills and have excellent communication skills.
