Overview
We are currently offering an exciting opportunity to join the Urgent Care Team (discharge team). We have two teams based at Worcester Royal Hospital and the Alexandra Hospital operating a 7-day service to support complex discharges both in the acute setting and in the community. There is potential to work weekends and bank holidays, and additional pay is available for bank holiday and Sunday working. The team work closely with colleagues from the acute trust, health and care trust and other WCC teams with a focus on the home first principle, strengths-based practice, and collaborative working, utilising the 3 Conversation model and working within the Care Act 2014. The team is committed to working with patients to establish their goals and aspirations and work towards returning patients to their own homes for as long as possible, working with the Mental Capacity Act 2005 to ensure least restrictive options and patient choice is at the heart of discharge.
As a Social Care Worker, you will be supported to manage your own case load to support complex hospital discharge, identifying any adults who may be at risk of abuse or neglect, completing MCA and working within a multi-disciplinary environment. The Urgent Care Team is a supportive team who offer regular supervision and reflection to develop your knowledge, skills, and practice. We encourage professional development and training opportunities to support this.
