Community Specialist Nurse – Intermediate Care
The Ealing Rapid Response/Admission Avoidance team is seeking a skilled Community Specialist Nurse to join our Rapid Response team, delivering high‑quality, patient‑centred care across community and inpatient settings.
Responsibilities
* Assess patients effectively and identify early signs of deterioration to ensure timely intervention.
* Deliver comprehensive care to vulnerable individuals using a range of clinical and non‑clinical equipment.
* Travel independently across borough and wider network to provide care.
* Maintain sustained concentration, critical thinking and analyse own practice and service performance.
* Manage challenging situations involving patients, families and colleagues with professionalism, empathy and integrity.
* Provide personal care while upholding dignity and safety at all times in varied environments.
* Participate in training and education sessions and develop in teaching roles.
Qualifications & Requirements
* Strong IT literacy and effective use of office systems.
* Confidence in working autonomously and managing duties independently.
* Physical capability to use clinical and non‑clinical equipment and provide comprehensive care.
* Ability to travel independently across borough and wider network.
* Emotionally resilient with capacity to handle demanding situations.
