Role Overview
The Rehabilitation at Home (RAH) Team supports adult patients following fracture neck of femur surgery from the Orthopaedic wards at Aintree Hospital. The team delivers intensive, short‑term therapy interventions in patients’ homes to maximise independence, working across the Liverpool city boundary, South Sefton and Knowsley in close collaboration with Orthopaedic wards and other stakeholders.
Responsibilities
* Lead day‑to‑day running of the RAH department, ensuring efficient, effective and responsive care.
* Provide line management of the clinical team, develop rotas and working patterns to maintain business continuity.
* Maintain clinical credibility by delivering evidence‑based practice and managing complex clinical situations.
* Ensure delivery of the Trust’s quality strategy, establishing systems and processes that reflect core objectives.
* Monitor clinical activity, performance and service budgets, producing reports and data for performance review.
* Support service development and modernisation in partnership with the service lead, including policy and research participation.
* Advocate for patients across professional forums, challenging attitudes and behaviours that impede care.
* Deliver holistic assessment of health needs, develop individualised care plans and implement care delivery with a focus on health promotion, self‑care and palliative aspects.
* Oversee risk management, incident reporting and support investigations as delegated by the service lead.
* Provide induction and mentoring for new team members, students and staff, addressing skill deficits and supporting professional development.
* Act as an authorised signatory for timecard and mileage claims, ensuring probity and budget compliance.
