Job overview

The purpose of the post is to support and enhance the Acute Medical services within the Trust. It is envisaged this post would be suitable for a Senior CMT or equivalent who is awaiting MRCP part 2 or early years registrar. The post holder will support the Acute on call rota, responsible for managing, co-ordinating and running the acute take smoothly and working alongside the Acute Medical Consultant / On call Consultant Physician of the day on MAU. This will involve taking part in evening shifts, night shifts and weekend working dependent upon experience of the candidate and the need of the service.

Main duties of the job

The job will contain the following exposure:

* 1 session per week of teaching University of Buckingham Medical School 1st and 2nd year students. (Systems based history and clinical examination skills.)
* Set sessions working as a registrar level doctor in the Medical Admission Unit (MAU)
* 1 session of SPA activity.
* Set sessions working in same day emergency care (SDEC) seeing new referrals to acute medicine and promoting community treatments.
* Full participation in the general medicine registrar on call rota including nights and weekends on a 1:16 basis.

Detailed job description and main responsibilities

Teaching

In one calendar year the 1st and 2nd year University of Buckingham Medical Students are present for 40 general medical sessions at Milton Keynes. Under the supervision of Consultant colleagues the candidate will be expected to deliver small group teaching focusing on history and clinical examination skills as dictated by the Buckingham syllabus. The successful candidate will also coordinate the delivery of acute medical junior doctor teaching on Friday lunchtime.

Medical Admissions Unit (Ward 1, MAU)

A daily consultant ward round is provide to a 28 bedded MAU / Short stay ward. The successful candidate will provide a set number of sessions of MAU registrar cover to provide backfill for the acute medical registrars to pursue other educational activities and to provide cover when ward registrars are away.

The successful candidate will be responsible to work under the supervision of the Consultant Physicians. He/she will be expected to participate in most of the day to day activities of the department in cooperation with the rest of the medicine staff. The successful candidate is advised to participate in all the other ongoing activity of the department and to attend various departmental teaching sessions and regional/national post graduate meetings.

The postholder will perform, with supervision, procedures appropriate for their level and experience. They will also be expected to be competent in all general areas of the specialty consistent with training and experience and to cover the work of colleagues as appropriate. They will be expected to actively participate in educational activities including teaching, grand rounds, case presentations and audit.

Same Day Emergency Care Unit (SDEC)

The successful candidate will have a set number of sessions per week based in the SDEC Unit. This session will involve working directly with the acute medical consultant delivering ambulatory care to medical patients streamed to the ED or via referral from general practice//ambulance services.

On Call

The general medial registrar on call is a full shift 1:14 – 1:16 on call rota 08:00 until 22:00 covering days, evenings, weekends and 20:00 until 09:00 covering nights. The successful candidate should be capable of performing in the role of Medical registrar on call, competent to run a cardiac arrest although may not yet have gained part 2 MRCP but MRCP is desirable.

Qualifications

* MBBS or equivalent medical qualification
* ALS
* MRCP 1
* MRCP Part 2 written or PACES
* MRCP 2
* Full registration with the GMC at time of appointment
* Evidence of significant general medicine experience

Fitness to practice

* Applicant’s knowledge is up to date and is fit to practice safely

Clinical Skills

* Relevant specialty clinical knowledge: capacity to apply sound clinical knowledge relevant to the job.
* Clinical judgment: experience in making clinical decisions and managing risk. Knows when to seek help, able to prioritise clinical need.
* Practical skills: shows aptitude for practical skills, required in the job
* Proven ability to work effectively in different clinical settings required in the job
* Evidence of experience in a range of acute medical specialties, with experience of managing patients on unselected medical take during core medical training or equivalent
* Able to work on the full Med Reg on-call rota (long days, nights and weekends)
