Overview
Job summary: The Registered Nursing Associate will join the Sudbury Community Health Team, based at the Sudbury Community Health Centre. The team includes a range of health professionals and services, enabling integrated care in the community.
The successful applicant will hold a foundation degree in nursing and will join a team of Community Nurses and Therapists. The role involves using nursing skills and may include providing therapy support to patients in the community. You will visit patients in their homes and assess patients within specified clinical areas, as identified by a registered practitioner. You will prepare for, carry out and monitor assessments and treatments in specified clinical areas, implementing the patient\u2019s individualised care plan developed with the team.
Main duties
You will work on site at the health centre and may be involved in delivering nursing care in specialist clinics. You will use a wide range of nursing skills, with strong assessment capabilities and the ability to provide safe, effective treatments in a community setting. Essential areas include holistic nursing assessments, supporting palliative care needs, wound management, diabetic care, catheter care and venepuncture. You will have regular clinical support but will be prepared to work predominantly independently on specified tasks and demonstrate basic clinical reasoning for treatment progressions. You will be involved in working with the MDT and integrating with social services, hospice and hospital staff, and be committed to ongoing learning and development. You may also act as mentor and role model to other support staff.

Job responsibilities
The role is to work with the multi professional team and the Registered Nurse, contributing to ongoing assessment of patients and providing and monitoring care aligned with care plans written and agreed by the Registered Nurse or therapist. You will be accountable for delivering compassionate, person-centred, evidence-based care, monitoring patient health needs continuously and escalating changes in a timely manner in partnership with the multi professional team.
Qualifications
* Essential: Registered Nursing Associate on the NMC register
* Essential: Nursing Associate foundation degree qualification
* Essential: GCSE A-C or equivalent in Maths and English or Functional Skills Level 2
* Desirable: Intermediate life support
Experience & Skills
* Essential: Experience of working in teams under supervision as part of a multi-disciplinary team
* Essential: Ability to evaluate own strengths and development needs and seek guidance
* Essential: Experience of communicating complex, sensitive information
* Desirable: Previous experience in a similar care setting
* Desirable: Experience in supervising and assessing students and new starters
