Job Description
We welcome applications to join our Community Team for Learning Disabilities in Carmarthen. We currently have a Full‑time Well‑being and Welfare Practitioner posts available.
You will be part of a supportive team committed to delivering high quality Social Work services and a wider service working together to support individuals and their families to feel connected to their communities. You will work with adults with learning disabilities as part of a multi‑disciplinary team to promote their well‑being and maximise their independence in regard to finances and welfare benefits.
The Community Learning Disabilities Team is committed to providing exceptional support and care for individuals with learning disabilities and their carers, with a dedication to improving outcomes through compassionate, person‑centred approaches.
Travel within the county is essential.
