Main duties of the job
We would like to hear from you if you are SCPHN Health Visitor or a SCPHN Health Visitor Student due to qualify. We actively encourage any applicants to come and meet the 0-19 team.
Our service offer:
Undertake initial family health needs assessments
Identify vulnerable families and mobilise appropriate resources.
Prioritise workload to reflect a service based on reducing health inequalities.
Manage workload in a corporate way within the integrated team.
Ensure effective and equitable management of workload within the team.
Be pro-active in multi-agency networking and collaboration to improve life chances for all children.
Identify actual or safeguarding concerns following Multi-Agency Code of Practice, CNWL safeguarding and child protection policies.
Attend safeguarding meetings as required o
Work in partnership with vulnerable families to enable optimum health gains.
Undertake child/family assessments and agree programme of child health screening and health promotion activity.
Carry out specific health and developmental screening where identified or delegate responsibility to other team members.
Facilitate group work for agreed health promotion programmes.
Other Information:
You should have a relevant qualification in nursing, and be a Specialist Community Public Health Nursing Practitioner (Health Visitor)

Detailed Job Description And Main Responsibilities
Undertake initial family health needs assessments or delegate the responsibility where appropriate to other members of the team.
Identify vulnerable families and mobilise appropriate resources.

Offer Health Visitor expertise to clients within a community setting.
Manage and develop skill mixed staff within the integrated team.
Identify actual or suspected child abuse. Follow Milton Keynes Multi-Agency Code of Practice and Community Health Services child protection polices.
Attend child protection conferences and core groups as required or delegate responsibility where appropriate to other members of the team.
Work in partnership with vulnerable families to enable optimum health gains. Undertake risk assessments prior to home visits.
Undertake child/family assessments and agree programme of child health screening and health promotion activity.

Knowledge & Skills
Essential criteria
Safeguarding children and understanding of associated local and national policy, procedure and practice
Awareness of local and national public health initiatives
Awareness of frameworks for accountability and issues of confidentiality
Excellent communication skills and the ability to adapt to a variety of situations
Confident in managing change
Competent in use of IT and associated programmes eg. Microsoft Teams, Systmone, Microsoft Office
Qualifications
Essential criteria
Specialist Community Public Health Nurse
Registered Nurse/Midwife with current NMC registration
Desirable criteria
Qualification in teaching/assessing clinical practice
Unicef Breastfeeding and Relationship Building training
Evidence of continuing professional development
Experience

Evidence of caseload management and office organisational skills
Experience of working with families and children to improve health outcomes through the delivery of the Healthy Child programme
