The Care Co-ordinator role is essential in ensuring the smooth running, and effective delivery of high-quality domiciliary and community-based care services. The Care Co-ordinator is responsible for care planning, staff allocation, rota management and effective communication within the organisation. -Key Responsibilities -Care Assessments and Planning - Support assessments for new and existing service users. - Maintain, update and review personalised care plans. - Staff Scheduling and Rota Management - Respond to staff absences and ensure continuity. - Service Delivery & Operational Support - Ensure shifts and visits run smoothly. Support the Registered Manager in daily operations. Quality Assurance and Compliance - Conduct spot checks, audits and client reviews. - Report incidents and safeguarding concerns. - Staff Support and Supervision Provide communication, guidance and updates to care staff. - Assist with performance monitoring. On-Call Responsibilities Participate in on-call rota and emergency response. Person Specification Essential: - Previous care coordination or scheduling experience. - Strong organisational and communication skills.
