The Care Co-ordinator is responsible for care planning, staff allocation, rota management and effective communication within the organisation.
* Key Responsibilities
* Care Assessments and Planning
* Support assessments for new and existing service users.
* Maintain, update and review personalised care plans.
* Staff Scheduling and Rota Management
* Respond to staff absences and ensure continuity.
* Service Delivery & Operational Support
* Ensure shifts and visits run smoothly.
Support the Registered Manager in daily operations.
Quality Assurance and Compliance
* Conduct spot checks, audits and client reviews.
* Report incidents and safeguarding concerns.
* Staff Support and Supervision
Provide communication, guidance and updates to care staff.
* Assist with performance monitoring.
On-Call Responsibilities
Participate in on-call rota and emergency response.
Person Specification
Essential:
* Previous care coordination or scheduling experience.
* Strong organisational and communication skills.
Desirable:
* NVQ Level 3.
* Experience in domiciliary care.
* Highly organised
* Manage own work with minimal supervision.
* Calm under pressure
