Job summary Closing date: 20th May 2026 (May Change) Shortlisting to take place: 21st May 2026 Interviews to commence: 28th May 2026 (May Change) The post-holder will be responsible for the management of a defined caseload of Children with Medical Complexity (CMC) and will provide and develop highly specialist nursing support, advice, education and/or care to that specific group. They will carry continuing responsibility for the assessment of care needs, the development, implementation and evaluation of programmes of care, including setting of standards of care, and take a lead role in establishing evidence-based practice within Children's Services. Main duties of the job Provide clear and effective leadership and co-ordination of the acute patient pathway for children with medical complexity (CMC). Work with minimal supervision to develop and deliver the specialist service based on principles that are evidence based and which enhance the quality of patient care. Ensure a high-quality service meeting the individual needs of the child and their family. Lead the co-ordination of holistic and seamless clinical care within acute services for CMC, who are often known to several specialty teams Promote efficient patient pathways Work collaboratively with all stakeholders including MDTs, CNSs, Clinical teams, Consultants, Therapists, and Community Nursing teams across the Trust/Health Board to facilitate improved patient flow, capacity and continuity of care. Effectively liaise with education and social care to reduce fragmentation of care across organisational boundaries. Co-ordinate multidisciplinary assessments meeting the physical, psychological, emotional and spiritual needs of the BCYP and their family at times of stability and crisis and together with the multidisciplinary team developing strategies to address these needs.
