Job Overview An exciting opportunity has arisen for a highly specialist Physiotherapist to be part of our Community Rehabilitation team within Ealing Short Term Rehabilitation. You will be able to provide decisive and effective therapy input and will demonstrate leadership qualities, excellent communication skills and a flexible approach when working under pressure. You will be passionate about the value of community rehabilitation and preventing hospital admissions. The successful candidate will be an experienced clinical leader with the ability to motivate and inspire others. They will have a proven track record of providing expectational care to service users and carers while supporting staff to deliver an excellent service. Main duties of the job As a senior member of the Therapy Team, you will be expected to be a practitioner with a broad range of clinical competencies able to adapt to a variety of clinical circumstances. You will be part of a community team where your proven ability to make high level clinical decisions will aid patient recovery and independence. You will provide complex assessments and make high impact clinical decisions. You will be expected to work autonomously whilst drawing assistance from other colleagues in primary and secondary care. You will use evidence based principles/current best practise and specialist skills to assess, plan, implement and evaluate clinical interventions and to ensure that all members of your team do so accordingly. Your exceptional caseload management and communication skills will enable you to prioritise patient care and delegate to junior members of your team. You will be expected to be a highly skilled Physiotherapist who is approachable, flexible, an excellent communicator, and able to adapt to the needs of both patients and service. Job Summary To provide a highly skilled Physiotherapy assessment and treatment for patients with complex conditions who are referred to the service. To plan, implement, evaluate treatment and management programmes to ensure the identified needs of the individual are met at all stages of the rehabilitation process or disease progression on a regular basis, further to educate and advise patients, their families and carers in order to facilitate self-management of their condition and optimise their rehabilitation potential and quality of life. To act as a source of expertise on the management of the complex disability and provide an advisory service to Physiotherapy colleagues, other health and social care professionals, GPs, and other involved agencies. To actively participate and support the development of new initiatives within ECP and CNWL. To actively support the continuing professional development of all staff, by providing expert advice and teaching. To manage, assist and supervise the junior staff. Smooth running and day-to-day management of the community therapy service. Person specification Education And Qualifications Essential criteria Bachelor Degree in Physiotherapy HCPC Registration Post Registration courses e.g.  Neurology  Older people/elderly rehab  Goal settin Desirable criteria Post-Graduate or Master's Degree Member of CSP Membership and/or involvement in a CSP Special interest group/ Professional Network Previous Experience Essential criteria Previous experience of working in the UK or a similar health economy as a physiotherapist. Previous experience to include the following; Frailty/Elderly rehab, Medicine and general surgery, Orthopaedics, Falls, Community working, Pain management, Neurology, Dementia, MSK, Respiratory, Experience supervising students and junior staff Experience of completion of audit and service developments Desirable criteria Experience of working as an autonomous practitioner in a community setting Other Essential criteria Ability to travel / work from sites across the Trust to fulfil the requirements of the post and carry equipment The ability to work under pressure and set priorities, delegate and manage own time and support the team to manage their caseloads Able to be part of the weekend working rota Desirable criteria Car owner and able to use car for work and carry equipment Skills and Knowledge Essential criteria Excellent complex written and verbal communication skills Demonstrates flexibility, complex problem solving, initiative and leadership skills. Awareness of developments in NHS and service delivery. Demonstration of knowledge of standards of practice, evidence and best practice guidelines. Knowledge and experience of Quality Improvement Understanding the principles of Multi-Disciplinary Teamwork Able to demonstrate clinical and professional development reflective practice Able to demonstrate a good understanding of role of the physiotherapist in rehabilitation as well as the role of other professionals in MDT Able to demonstrate insight into the problems of disability and its effects Able to de-escalate complex situations and demonstrates conflict resolution skills Desirable criteria Excellent IT skills including Outlook, Word, Excel, database s etc
