Main duties of the job

The post holder is responsible for the delivery ofgeneral practice nursing to the whole practice population. The role encompassestwo main roles; the delivery of evidence-based practice for patients presentingwith a long term condition and the provision of preventative health care to thepractice population.

As an autonomous practitioner the nurse is responsiblefor the care delivered and must demonstrate critical thinking and skills inclinical decision making when managing patients. The practice nurse will workcollaboratively within the general practice team to meet the needs of patientsand support the delivery of policy and procedures.

The Practice has always prided itself on providing high quality care in a traditional family practice setting. We have built a positive relationship with our consultant colleagues at UHD. Two of our partners are GP trainers, and learning and education is an important part of what we do. One of our partners is Executive Lead for Integrated Care at UHD and we are heavily involved in network development.

We are a SystemOne practice with approximately 9500 patients, based in purpose built premises.


Job responsibilities

Duties and Responsibilities

Welcoming patients in a friendly manner and maintaining patient privacy and respect.

Administrative Responsibilities:

Assist in developing the treatment room to meet the health needs of the patient population efficiently and effectively

Assist in the implementation of the Quality and Outcomes Framework (QOF) and

Enhanced Services contracts

Audit activity related to QOF and Enhanced Services

Implement research activities and conduct research studies as and when required

Clinical Practice:

* Educate, advise or counsel patients and their families on health issues and always ensure delivery of opportunistic health promotion
* Support patients to adopt health promotion strategies that promote patients to live healthily and promote principles of self-care
* Support and manage health needs of women presenting for cervical cytology consultations
* Support and advise women requesting information relating to family planning needs
* Provision of travel advice and administration of vaccinations where appropriate for patients travelling abroad
* Implement and participate in vaccination and immunisation programmes for both adults and children and ordering of vaccines to maintain stock levels
* Promote and deliver evidence-based care for patients presenting with aural conditions
* Recognise abnormalities.
* Assess, plan, implement and evaluate individual treatment plans for individuals with long term conditions, particularly diabetes/Asthma/COPD.
* For patients with the above conditions, review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice, national and practice protocols
* Proactively identify and manage treatment plans for patients at risk of developing a long-term condition as appropriate
* Work alongside patients to support adherence to prescribed medications and appropriate treatments, using a holistic patient centred approach.
* Meet the continuing needs of patients presenting for wound care

Recognising issues and gathering sufficient information to refer (e.g. drugs;

domestic violence; child protection; vulnerable adults; senior patients; social problems)

Further Clinical Duties:

* Chaperoning and assisting patients who are being examined by another clinician
* Assisting GPs with minor surgery and gynaecological procedures
* Venepuncture
* Dopplers
* Removal of sutures/clips
* Performing ECGs
* New patient health checks
* Administration of injections
* Assist with re-stocking and maintaining clinical areas and consulting rooms in absence of HCA
* Checking of emergency trolley, defibrillator, oxygen cylinder in absence of HCA

Communication:

* Demonstrate sensitive communication styles to ensure patients are fully informed and consent to treatment
* Communicate with and support patients who have received bad news or suffered a bereavement
* Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating
* Anticipate barriers to communication and take action to improve communication
* Maintain effective communication with individuals and groups within the practice environment
* Act as an advocate for patient and colleagues

Person Specification


Qualifications

* Registered Nurse (Nursing and Midwifery Council)
* Mentor or teaching qualification
* Post graduate diploma or degree (Chronic Disease Management)
* Family planning qualification

Experience

* Experience of working in a primary care environment
* Experience of working autonomously
* Experience of infection prevention and control measures
* Experience of working as a practice nurse or community nurse
* Experience of managing elderly and housebound patients
* Experience of quality initiatives, i.e., benchmarking


Clinical Knowledge and Skills

* Clinical knowledge in the following:
* Wound care/removal of sutures and staples
* ECGs
* Venepuncture
* New patient medicals
* Immunisations (routine, childhood and travel)
* Requesting pathology tests and processing the results, advising patients accordingly
* Chronic disease management awareness for:
* Diabetes
* CHD
* Asthma
* Understanding the importance of evidence-based practice
* Understand the requirement for PGDs and associated policy
* Ability to record accurate clinical notes
* Ability to work within own scope of practice and understanding when to refer to GPs
* Chaperone procedure
* Broad knowledge of clinical governance
* Knowledge of public health issues in the local area
* Awareness of issues within the wider health arena
