Overview
Following a hospital stay, the ethos across Bristol, North Somerset and South Gloucestershire (BNSSG) for all people being discharged is 'Home is Best'. For some service users with complex needs or requiring a period of further assessment, discharge to a complex assessment bed (generally in a care home) may be appropriate. During their stay, service users will be supported, along with their carers and families, around complex discharge planning to enable a timely and safe discharge to the person’s usual residence or to a more suitable location if appropriate. A multi-disciplinary model is actively promoted, with staff working closely with social workers, GPs and other Acute and Community colleagues to ensure the person we support is at the heart of every decision.
Responsibilities
* Promote optimum independence by encouraging rehabilitation techniques.
* As a rehab support worker, you will be a key figure in completing day-to-day rehabilitation with patients under the instruction of lead therapists, provide feedback on progress, and help service users achieve their maximum potential.
* Undertake a range of delegated clinical and therapy-led activities to support service users on the caseload.
* Work under direct supervision and be responsible to senior therapists, providing one-to-one and group rehabilitation.
* Deliver a full range of health and social care activities within the Adult Service, supported by a robust competency framework for adults in complex assessment beds.
* Be part of an integrated multi-disciplinary team (MDT) including nurses, physiotherapists, occupational therapists and other Allied Health Professionals.
* Operate within the Service hours (08:00–18:00) with flexible working discussed and potential weekend cover in the future.
* Occasionally provide organisational and service resilience by supporting other teams or services within the locality and Sirona wide on a rota basis when needed.
