Job summary
We are looking for a Practice Nurse to join our small, but dedicated team here at Stuart Road Surgery. The position is for 16 to 20 hours per week. Salary is dependent upon experience. Please state salary expectations when applying for the position.

Main duties of the job
To be fully proficient in Chronic disease management,vaccinations and immunisations, cervical screening, contraceptive reviews,wound care including compression, as well as other general treatment roomduties.
To provide support to junior members of the nursing teamincluding healthcare assistants and will direct their work to ensure that thePractices provide high quality care.
You will oversee infection control standards and cold chaincompliancy.
To work within NMC Code of Conduct and to agreed Practicestandards and protocols
Oversee recall systems for chronic diseases, cervicalscreening, immunisations, and other key areas as relevant to Practice Nursing
Delivery effective implementation of management of patientswith Chronic Diseases as detailed by the Quality and Outcomes Framework, localquality schemes and national quality schemes
Support the implementation of evidence-based guidelines inconjunction with the wider team
Encourage the development of integrated services withinPrimary Care to include integrated practice-based teams and multidisciplinaryworking
Oversee infection control and cold chain systems to ensurecompliancy
Ensure that all nurse-related Care Quality Commissionevidence table indicators are compliant, demonstrating evidence, review, andaudit cycle
About us
We have a small, but mighty workforce consisting of two GP partners, five salaried GP's, two HCA's and PCN appointed ARRS staff who work both on and off site at our surgery; all supported by our Practice / Business Manager, Assistant Practice Manager and our reception/admin team.
Job responsibilities
Participate in an annual appraisal
Actively participate in clinical and practice meetings
Develops clinical protocols for the surgery and the team.
Clinical duties
Provide Chronic disease management for Diabetes, COPD,Asthma, and CHD/CVD
Perform cervical screening and promote breast awareness
Administer adult and paediatric injections
Provide adult and child immunisations ensuring fullachievement of targets
Ensure safe storage, rotation, and disposal of vaccines anddrugs
Undertake venepuncture and electrocardiography
Undertake wound care/ management / dressing techniques
Undertake leg ulcer management, dopplers and compressionbandaging
Provide advice on lifestyle issues, assisting and supportingpatients in changing their unhealthy behaviours and refer appropriately
Participate in the smoking cessation service
Deliver travel health care, including vaccinations andadvice for patients
Recognise the signs of child abuse, family violence,vulnerable adults, substance abuse, addictive behaviours, suicidal tendencies,and be aware of local, referral procedures
Work with safeguarding teams where relevant
Ensure that current evidence-based approaches to patientcare are standard practice
Apply infection control measures when collecting andhandling of laboratory specimens, disposing of waste materials, dealing withblood and body fluids spillages and report and treat sharps injuries
Undertake first aid and management of emergencies, e.g.burns, haemorrhages, hypo/hyperglycaemic emergencies/asthma attacks
Provides a comprehensive family planning service
Audit outcomes of care against standards and initiate changeas necessary ensuring follow-up audit cycle
Maintain and update disease registers
Person Specification
Qualifications
* Nursing degree or other relevant nursing qualification granting professional license to practice primary care nursing
* Qualified in chronic disease management for Diabetes, Asthma, COPD and CHD/CVD
* Wound dressing and leg compressions
* Venepuncture and electrocardiography
* Previous experience of Care Quality Commission evidence table to achieve nursing related indicators to a high standard.
* Stable career history showing clear commitment and progression
Experience

* Highly motivated
* Excellent telephone and bedside manner
* Demonstrable problem solving experience
* Highly experienced Practice Nurse Experienced in delivering chronic disease management Experienced in delivering cervical screening Experienced in vaccinations, injections, and immunisations Experienced in wound care management, dressing techniques, leg ulcer management and dopplers.
* Experience in overseeing infection control systems to achieve compliancy
* Experienced in maintaining cold chain compliancy
* Experience of using SystmOne
