What you will do: * Carry out needs-led assessments of occupational performance for clients with a wide range of medical conditions and physical disabilities. * Develop individual goal-oriented action plans, considering clients' physiological, psychological, and environmental factors. * Consult clients and their carers to agree on appropriate provision of equipment and adaptations. * Lead initiatives across organisational boundaries to deliver integrated housing solutions for adults with complex needs. * Conduct holistic assessments to manage and reduce risk in clients' homes, promoting safe independence. * Manage your own complex clinical caseload autonomously while providing clinical advice to colleagues. * Diagnose and address complex manual handling situations, offering expert training and advice. * Support the Adult Contact Team in identifying eligible needs and providing robust signposting. * Promote the use of assisted technology like Telecare to encourage client independence.
