We are looking for a committed and caring treatment room nurse with a specialist interest in asthma and COPD to join our friendly clinical team.

General Practice Nurse (GPN)
Rate of Pay
Negotiable depending upon experience.
Key Responsibilities
* Provide evidence‑based general practice nursing to the whole practice population.
* Deliver care for patients with asthma, COPD and other chronic conditions, including assessment, planning, implementation and evaluation of treatment programmes.
* Identify and manage treatment plans for patients at risk of developing chronic conditions; collaborate with other healthcare professionals for multi‑disciplinary care.
* Review medication for therapeutic effectiveness, ensuring adherence and providing information on side effects and drug interactions.
* Provide patient education and support for lifestyle changes, health promotion, self‑care strategies and preventative health measures.
* Support women with family planning and cervical cytology consultations, and manage mental health needs in accordance with national guidance.
* Implement and participate in vaccination and immunisation programmes for adults and children.
* Deliver care for aural conditions, wound care, and other treatment room needs.
* Communicate sensitively, obtain informed consent, and support patients with bad news, ensuring effective communication across cultural and language barriers.
* Maintain accurate, contemporary patient records in compliance with legislation and NHS policies.
* Lead and participate in quality improvement activities, including audit cycles, quality governance systems, and performance monitoring.
* Manage departmental risks, infection control, vaccination storage, and controlled drug usage in line with legal requirements.
* Provide supervision and training to Healthcare Assistants, students, and other staff; act as a positive role model and clinical leader.
* Support the recruitment and staff planning with the Nurse Manager to ensure appropriate staffing levels and skill mix.
* Participate in local guideline development, commissioning projects, and community outreach initiatives.
* Apply technology to support practice management, data coding, and information sharing while safeguarding patient confidentiality and GDPR compliance.
Qualifications
* Knowledge of management of patients with chronic conditions.
* Accountability of own role and other roles in a nurse‑led service.
* Knowledge of health‑promotion strategies.
* Knowledge of local and national health policy.
* Knowledge of clinical governance issues in primary care.
* Knowledge of patient group directions and associated policy.
* Clinical skills in cervical cytology, immunisation, vaccination, wound care (including compression bandaging) and ear care.
* Change‑management skills and ability to support patients to change lifestyle.
* Written and verbal communication skills.
* Ability to communicate difficult messages to patients and families.
* Negotiation and conflict resolution skills.
* At least 2 years recent primary or community nursing experience.
* Experience of nurse‑led management of chronic conditions.
* Experience of implementing protocols and clinical guidelines.
* Experience of quality initiatives such as clinical audits.
* Practice Assessor/Practice Supervisor training and experience.
* Highly motivated, flexible and a team player.
* Knowledge of public health issues and ability to identify determinants of health in the local area.
* Clinical leadership skills and ability to provide teaching and supervision in a clinical setting.
Experience
* At least 2 years recent primary or community nursing experience.
* Experience of nurse‑led management of chronic conditions.
* Experience of implementing protocols and clinical guidelines.
* Experience of quality initiatives such as clinical audits.
* Primary care experience.
