Salary: £39,144 - £41,110 per annum (Inclusive of 5% Market Supplement)

About the role
Join us as a Level 2 Social Worker and collaborate with health colleagues in a multi‑agency integrated service focused on Health and Social Care. The aim of this role is to reduce unnecessary social hospital admissions, support carer breakdowns, and promote independence by assessing people in their homes and assisting them to maximise independent living. Our focus includes community support before hospital transfer and assistance in Accident and Emergency departments.
Responsibilities
* Assess needs for eligibility and devise care plans.
* Achieve or maintain well‑being outcomes for individuals.
* Assist the manager in delivering a holistic service.
* Ensure the safety of vulnerable adults.
* Facilitate prompt assistance for adults and their caregivers during crises such as falls or caregiver emergencies.
Qualifications
* Registered with Social Work England.
* Strong interpersonal and communication skills.
* Updated knowledge of relevant legislation.
* In‑depth understanding of relevant adult social care legislation.
