THIS POST IS FIXED TERM/SECONDMENT FOR 9 MONTHS TO MEET THE NEEDS OF THE SERVICE.

An opportunity has occurred to join the District Nursing Service in the Dwyfor and Meirionnydd Localities. The role will involve supporting the district nursing service with the Eifionydd District Nursing team and the Out of Hours team to care for people living in the community who require nursing care. The post will include night shift rotation as per rota. The successful candidate will have obtained or be working towards NVQ Level III. Welsh speaking is desirable.
Main duties include direct patient care allocated by a trained nurse, organising tasks by priority, assisting patients to maintain an acceptable quality of life, maintaining accurate documentation, ordering and checking supplies, and assisting with clerical duties.
Travel between patients’ homes within towns and rural areas is required.

Job responsibilities
* Direct patient care as allocated by a trained nurse, within the care plan, and reassessed by a registered nurse. Organise tasks prospectively according to priority.
* Assist and support the patient in maintaining an acceptable quality of life.
* Maintain accurate and legible documentation in line with Health Board policy, ensuring confidentiality at all times.
* Order HSDU, stores, check delivery, ensuring efficient use of resources.
* Assist with any appropriate clerical duties.
The ability to speak Welsh is desirable.
