What you will do
* Collaborate within a fast-paced dynamic team to support discharges from acute and community hospitals.
* Work to prevent unnecessary admissions to hospitals by providing effective occupational therapy interventions.
* Conduct comprehensive assessments and develop tailored reablement plans for individuals you serve.
* Serve as a key communicator between multidisciplinary teams and community resources, ensuring seamless service delivery.
* Utilize your expertise to adapt environments, train carers and patients, and facilitate safe and efficient discharges.
Requirements of the Occupational Therapist
* Degree or equivalent in Occupational Therapy
* Recent UK experience working as an Occupational Therapist
* Recent NHS experience
* Registered with the HCPC
* Possess community and rehabilitation experience as a minimum
* Car driver; being adaptable and willing to travel as required to deliver services where needed
