About the Role
We are expanding our innovative Rapid Response & Intermediate Care (RRIC) service across Buckinghamshire. You will work with our Aylesbury team across Intermediate Care and Community Physiotherapy.
Responsibilities
* Provide expert interventions and rehabilitation as part of a dynamic team responding quickly to crises.
* Help patients stay in their own homes, supporting recovery, early hospital discharge, and end-of-life care.
* Contribute to triage, assessment, care planning, and case management of patients and carers.
* Collaborate with multidisciplinary team members including Physiotherapists, Nurses, Paramedics, Pharmacists, and Healthcare Assistants.
* Support development of clinical pathways and work across Intermediate Care and Physiotherapy pathways.
