Position Overview
We are currently seeking dedicated Physician Assistants and Nurse Practitioners to provide exceptional care at our skilled nursing facilities (SNFs). With a variety of roles available, we cater to providers at different stages of their careers, offering flexible scheduling in a supportive work environment.
Position Type: Part Time, Exempt
Job Location: In-person
Shift Structure
* In-person rounding shifts typically start around 7 AM local time, with reasonable flexibility.
* Shifts are available on Saturdays, Sundays, and major holidays.
* This role covers weekend needs and two holidays per calendar year.
* Charting and documentation can be completed off-site.
Essential Functions and Responsibilities
* Manage clinical care and rounding for residents at assigned SNFs.
* Prescribe medications and treatments for patients.
* Deliver high-quality, patient-centered primary care services.
* Ensure timely documentation of all patient encounters.
* Provide consultations via phone or secure messaging with facility nursing staff.
* Utilize Theoria's proprietary electronic health record (EHR) system, ChartEasy™.
* Familiarity with PointClickCare, MatrixCare, and American HealthTech is a plus.
Physical Requirements
* Ability to move briskly within the facility to attend to patient needs.
* Punctual in-person attendance is required, typically between 7 AM and 1 PM.
* Proficient in using app-based EMR systems on mobile devices.
* Ability to navigate stairs and reach for items in various locations.
* Effective communication skills with patients and facility staff.
* Ability to observe details closely for accurate medical recommendations.
* Must be able to lift or move at least 25 pounds.
Requirements and Qualifications
* Valid PA/NP license in the state of practice.
* NP/PA degree from an accredited institution.
* Active DEA license (Theoria will assist new graduates with their application).
* Preferred experience or knowledge in Post-Acute/SNF care.
* Familiarity with EMR systems and charting practices.
