Join to apply for the Section 117 Complex Case Manager role at NHS.

An exciting opportunity has arisen with the Humber and North Yorkshire ICB for a qualified RMN/RNLD, or Social Worker, to join the Section 117 case management team covering North Yorkshire and York. We are looking to recruit a dynamic and innovative Band 6 Complex Case Manager to work as part of the team, delivering effective case management, assessment and reviews across the geography for persons who may be eligible for Section 117 funding. The post holder will review and case manage patients that are subject to Section 117 aftercare (Mental Health Act 1983) commissioning and people with learning disabilities and/or Autism ensuring that the services commissioned are appropriate and safe to manage service user care.

The successful candidate will join a newly formed integrated team of clinicians, social workers and administrative staff. The post holder will work independently, engage with multidisciplinary professionals, service users and carers, and ensure a clear understanding of case management and programme of care needs on a continuous basis. This includes discussing potential transfers to alternative placements with service users when necessary, including people living in the community and in Specialist placements, both in and out of area. The post holder would be a resource to support service providers with knowledge of alternative placements and treatment options.


Responsibilities

* Deliver the end-to-end Section 117 pathway, including eligibility assessment, review, case management and duty.
* Collaborate with a multidisciplinary team to ensure appropriate and safe care planning and service provision.
* Engage with families, local authority partners and the wider MDT; support service providers with knowledge of alternative placements and treatment options.
* Review and case manage patients subject to S117 aftercare and transforming care; ensure placements provide appropriate, good quality services and value for money.
* Liaise with Legal team regarding DoLs/CoP inquiries; reassess new patient placements within the initial 12 weeks per local policy.
* Manage highly complex, sensitive information during interactions with legal representatives and independent provider clinical teams, maintaining a calm and balanced approach.
* Attend service users clinical/multi-agency reviews and provide advice for complex Section 117 aftercare and CTR pre-discharge meetings; help ensure least restrictive placements.
* Attend Mental Health Review Tribunals (MHRT) when required to provide update reports on behalf of the ICB.


Qualifications and Experience

* Essential — Professional qualification in Nursing (RMN/RNLD) or Social Worker; degree level or equivalent experience; evidence of relevant registration; understanding of MHA Section 117/MCA, capacity and consent; safeguarding knowledge.
* Essential — Demonstrable Nursing/Social Work experience at band 5 or higher; experience of nursing relevant care groups and joint working with local authority and independent sector; clinical assessment experience; strong teamwork.
* Sound knowledge of the Mental Health Act (1983) and Mental Capacity Act (2005); excellent organisational skills; ability to travel as required by the role.


Skills and Attributes

* Experience working across organisational and external boundaries; safeguarding awareness; ability to manage complex, sensitive information.
* Strong communication and liaison skills with service users, families and professionals.

* Travel is required as part of the role.
* Disclosure and Barring Service check applies; post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions Order 1975).
* Applicants must have current UK professional registration.
* Salary: £38,682 to £46,580 per year; Band 6; Permanent; Full-time or Flexible working.
