We are looking for experienced Social Workers to join our Locality Team in Adult Social Care. Our mission is to empower adults to live independently as possible, and we need your expertise to make it happen. As a Social Worker working within the Localities team, your responsibilities will include:
1. Undertaking Care Act Assessments - identify individuals’ care and support needs, ensuring a personalised approach that emphasises the best outcomes
2. Completing Care Act Reviews - evaluating the strength and suitability of an individual's care and support plan, ensuring that it continues to meet their needs and preferences
3. Managing Safeguarding enquiries - investigating concerns about potential abuse or neglect, ensuring the safety and well-being of vulnerable individuals.
4. Mental Capacity assessments - determine an individual's ability to make specific decisions regarding their care and treatment.
5. Undertaking duty on rota basis
6. Undertaking DSTS and CHC
7. Court related work
Our teams are renowned for its ability to craft outstanding care plans that prioritise the needs and voices of service users, integrating input from various professionals, families/carers, and external agencies to ensure the best outcomes are achieved.
This position offers an excellent opportunity for individuals with outstanding communication skills and a compassionate manner capable of effectively managing high levels of emotional distress. In addition, we would expect the following from all suitable candidates:
8. To be registered with Social Work England and hold a degree in Social Care or similar.
9. At least 2 years post ASYE working experience as a UK registered social worker
10. Have experience of working in a multidisciplinary team setting.
11. Experience in undertaking complex Mental health and Physical health cases.
12. Have sound knowledge and ability to deliver social work support to service users and their families.
13. Ability to communicate complex social issues clearly to both patients and professionals.
14. Ability to plan and coordinate social care plans and aspects of patient discharge.

There are four Locality teams currently in place, aligned to the four Primary Care Network (PCN) footprint - Wallington, Carshalton, Sutton & Cheam. These services are for older people, people with physical and sensory disabilities and people with mental health related needs. Locality teams bring Social Workers together with NHS colleagues, supporting holistic, integrated working.
