We are looking for an enthusiastic salaried GP witha view to future partnership - to join our friendly, innovative practice ofjust under 7500 patients located in Maswell Park Health Centre, situated on theedge of Hounslow and Richmond boroughs. The post is for 4-6 sessions a week, ata session rate starting at £11,500 per annum pro rata, days and times can benegotiated.

Our clinical team is made up of doctors, nurses,and HCAs, and are very well supported by our management, admin and receptionteam. As a practice we strive to provide forward thinking, compassionate carefor our patients and their families. We are mindful of maintaining a healthywork balance and have adopted largely 15 minute appointments, a pharmacyservice to manage our repeat prescribing and embedded a document workflow teamto reduce admin burden on the clinical team. We are members of a proactive PCNand work closely with the pharmacists, first contact physiotherapists andsocial prescribers attached to our network. We are an approved trainingpractice with undergraduate teaching with Imperial Medical School, supportingstudents in years 1 and 2 for placement. We are keen that the successfulcandidate has an interested in education, continuity of care and project workand is looking for a long term commitment.

Main duties of the job

Main Duties

Clinical responsibilities

1. Assess and triagepatients, including same day triage, and as appropriate provide definitivetreatment (including prescribing medications following policy, NICE (national)and local clinical guidelines and local care pathways) or make necessaryreferrals to other members of the primary care team

2. Implement andevaluate treatment/interventions and care for patients presenting with anundifferentiated diagnosis

3. Clinically examineand assess patient needs including history taking, assess, diagnosis, plan,implement and evaluate interventions/treatments for patients with complex needsand onward refer where appropriate

4. Use complexdecision making to inform the diagnosis, investigation, complete management ofepisodes of care within a broad scope of practice

5. Prescribe andreview medication for therapeutic effectiveness, appropriate to patient needsand in accordance with evidence-based practice and national and practiceprotocols, and within scope of practice

6. Assess, identify,and refer patients presenting with mental health needs in accordance with theNSF for Mental Health Implement and participate in vaccination and immunisationprogrammes for both adults and children

7. Support thedelivery of advanced care plans

8. Recognise and workwithin your own competence and in accordance with professional codes ofconduct, maintaining accurate and contemporaneous health records


About us

We are an active member of Hounslow PCN whichconsists of 10 GP practices caring for a population of nearly 100,000 patients.

We work together with a range of local providers,including acute trusts, community services, social care, voluntary sector andpatient participation groups, to offer more personalised, co-ordinated healthand social care to our local population.

Delivering a quality service

Job responsibilities

Job Summary:

The post-holder will manage a caseload and dealwith a wide range of health needs in a primary care setting, ensuring thehighest standards of care for all registered and temporary patients.

Clinical Responsibilities:

In accordance with the Practice timetable, as agreed, the post-holderwill make him/herself available to undertake a variety of duties includingsurgery consultations, online consultations, telephone consultations andqueries, visiting patients at home, checking and signing repeat prescriptionsand dealing with queries, paperwork and correspondence in a timely fashion

Making professional, autonomous decisions in relation to presentingproblems, whether self-referred or referred from other health care workerswithin the organisation

Assessing the health care needs of patients with undifferentiated andundiagnosed problems

Screening patients for disease risk factors and early signs of illness

In consultation with patients and in line with current Practice diseasemanagement protocols, developing care plans for health

Providing counselling and health education

Admitting or discharging patients to and from the caseload and referringto other care providers as appropriate

Recording clear and contemporaneous consultation notes to agreedstandards

Compiling and issuing computer-generated acute and repeat prescriptions

Prescribing in accordance with the Practice prescribing formulary (orgenerically) whenever this is clinically appropriate

In general, the post-holder will be expected to undertake all the normalduties and responsibilities associated with a GP working within primary care.

Other Responsibilities within the Organisation:

Awareness of and compliance with all relevant Practicepolicies/guidelines, e.g. prescribing, confidentiality, data protection, healthand safety

A commitment to life-long learning and audit to ensure evidence-basedbest practice

Contributing to evaluation/audit and clinical standard setting withinthe organisation

Contributing to the development of computer-based patient records

Contributing to the summarising of patient records and Read-Codingpatient data

Attending training and events organised by the Practice or otheragencies, where appropriate.
