Part time, 22.2, hours per week (3 days) permanent contract, based in the Greenway Centre, Southmead.
An exciting opportunity has arisen within the Bristol Mental Health Partnership, which involves collaborative working between a number of organisations to include Second Step and Avon and Wiltshire Partnership Trust (part of the National Health Service).
The job opportunity is for that of a Second Step Facilitated Discharge Worker, based within the North Bristol Crisis Service. The North Crisis Team is one of three established crisis teams in the Bristol area, with the key remit of our service being to support people in avoiding hospital admissions, provide intensive home treatment to those at risk of hospital and to support people in their transition home following a stay in an acute psychiatric hospital.
Facilitated Discharge Workers are responsible for acting as the link for inpatient wards, community teams and supporting safe discharges, while maintaining a safe level of risk.
You will liaise with service users and ward staff prior to discharge to identify potential barriers and reduce the length of admission.
The successful applicant will have the opportunity to:
* integrate within our multi-disciplinary functioning team and will be integral to the “facilitating discharge from hospital” function of the team.
* will also have opportunity to spend time on the several of our local acute psychiatric wards within the Bristol area, meeting with our service users and supporting them in feeling safe, well and ready for a discharge to their home environment. It might also include supporting a person in removing any social barriers to any discharge
Some of the key skills will be particularly important to an individual applying for this post might include having a considered, thoughtful and reflective approach toward supporting an individual in their recovery journey, able to care plan and carry through a person’s care. It will also require a person to have good communication skills and an ability to liaise with multiple agencies (secondary care teams, GP’s friends and family are just a few examples of the people somebody might liaise with regularly).
The ideal candidate will have worked within mental health, providing a wide range of interventions to assist people, during a period of acute mental health crisis and connecting into community resources.
You must hold a current driving license, and have access to a vehicle.
