Trinity247 Care Services is seeking an organised and proactive Care Coordinator to join our team in Jersey. You will play a key role in ensuring the smooth delivery of care services by coordinating care staff, managing schedules, and maintaining excellent communication with clients, families, and care workers.
Requirements
• Previous experience in a care coordination or similar role (preferred)
