Overview
The Leeds and York Partnership NHS Foundation Trust (LYPFT) is seeking a full-time substantive Band 6 Learning Disability Nurse to join the Assessment and Referral Team (ART) within the Community Learning Disability Teams (CLDT). The role is based at St Marys Hospital and line-managed within the West and South CLDT. The service operates Monday to Friday, 9am to 5pm.
Responsibilities
* Work within a Multidisciplinary Team (MDT) to triage referrals into the CLDT and determine eligibility for Learning Disability specialist services.
* Gather comprehensive information from service users, carers, professionals and agencies regarding the referral and presenting needs.
* Discuss triage information in the MDT and make recommendations where LD services are not appropriate; opportunities to undertake direct face-to-face visits and assessments.
* Provide clinical and operational supervision to junior colleagues and contribute to the development of intervention tools and nursing resources.
* Lead or contribute to service development, projects and task groups; participate in robust supervision structures (1:1 clinical and managerial), weekly MDT meetings, and community nurse meetings.
* Coordinate across multi-disciplinary and multi-agency teams to advocate for the health needs of adults with learning disabilities; promote social inclusion in a diverse population.
Requirements / Qualifications
* Registered Nurse with Band 6 level experience in Learning Disability services; capable of autonomous clinical decision-making and presenting at MDTs.
* Passion for improving health and well-being of adults with a learning disability; experience in leading and delivering high-quality care.
* Ability to work autonomously, coordinate across multi-disciplinary and multi-agency teams, and lead on service development and projects.
* Commitment to values-based recruitment; experience with diverse populations and promoting social inclusion preferred.
* Eligibility and visa sponsorship requirements: Trust sponsorship is provided for registered healthcare practitioner roles; not available for Healthcare Support Workers; ensure compliance with Home Office requirements.
