Overview
We have a Team Manager (TM) role available in the Transitions Team covering the whole county. Your role will be to lead a team dedicated to supporting young people and their families through the transition from Children’s and Education services into Adult Social Care services, ensuring that person‑centred assessments and plans are undertaken in a timely way and that there is a strong focus on supporting young people to become more independent.
About the Role
Adult Social Care Team Managers are responsible for the day‑to‑day operational management of the following key areas:
* Developing a strength‑based culture in practice, leadership and coaching of the team
* Compliance with all statutory social care requirements including the Care Act, the Mental Health Act, the Mental Capacity Act and Buckinghamshire Multi‑agency Safeguarding procedures
* Contributing to service improvement to deliver outstanding support to Buckinghamshire residents
* Contributing to budget and financial management
* Performance against statutory and locally determined indicators
* Partnership working with internal and external stakeholders, particularly Children’s services and SEND teams
* Working with internal and external partners to deliver the Service Plan
Contract: Secondment/fixed term contract opportunity for 2 years.
About You
As a registered Social Worker or Occupational Therapist you will bring in‑depth knowledge of legislation and statutory guidance and be familiar with working to a strength‑based approach with particular experience and skills in working with young people.
As a leader you will be able to maintain a proactive culture within a team and work with colleagues and partners to support team development, design and implement new practices, and manage performance requirements.
As a person you will show resilience, persistence and a supportive and adaptable approach in working in an outcome focused way with our partners such as education, health services, community sector, housing, care providers and other stakeholders.
