Overview
The Hospital at Home (H@H) service, based at Lorn and the Isles Hospital, covers the Oban and Lorn area. It is a developing service that provides acute care at home as a safe alternative to hospital admission. The service aims to reduce hospital admission and length of stay by expanding clinical pathways and managing patients with both acute and long-term conditions at home.
The post is fixed term or possibly a secondment until March 2026. The role involves supporting the clinical staff with clinical tasks to ensure the smooth running of the service and to provide appropriate care to patients.
Responsibilities
* Provide clinical support to the Hospital at Home team to manage patients with acute and long-term conditions at home.
* Assist with clinical tasks as directed by registered clinicians.
* Support the smooth running of the service and patient care.
* Engage with patients and families to ensure safe and effective care at home.
Qualifications and Skills
* Health Care Support Worker experience or equivalent preferred.
* Ability to work effectively as part of a multidisciplinary team.
* Good communication and organisational skills; ability to adapt to a developing service.
