An exciting opportunity is available for a qualified Advanced Clinical Practitioner to join our growing Virtual Ward ACP team, delivering a high-quality hospital-at-home service across Shropshire, Telford and Wrekin.
You will work within a dynamic and innovative service delivering pathways in:
General Medicine
Acute Respiratory Infection
Heart Failure
Frailty
Surgery
This fixed term contract / secondment offers a fantastic chance to further develop advanced practice skills within a supportive multidisciplinary environment supported by consultant physicians

Main duties of the job
The post holder is required to deliver an advanced level of clinical practice autonomously in the fields of general medicine, Frailty, Respiratory, and Cardiology for patients who are supported by the team as an alternative to hospital admission.
The role will ensure that consistent, high quality, person-centred care is provided daily through the application of advanced clinical judgments and critical thinking to assess, investigate, diagnose, prescribe, plan, implement and evaluate clinical care in line with the National Framework for Advanced Clinical Practice. Embracing the four pillars of clinical practice; leadership and management, education and research.
