Overview
A permanent Advanced Clinical Practitioner (ACP) role at Ellesmere Port Hospital, part of the ACP team consisting of 2 ACPs and 1 Nurse Consultant, responsible for managing the two rehabilitation wards and the stroke rehabilitation ward.
Responsibilities
* Work autonomously to provide clinical care, expert advice, and leadership within the rehabilitation environment.
* Make independent clinical decisions and create treatment plans for both acute and chronic illnesses.
* Collaborate with multidisciplinary teams to deliver effective, efficient patient care, rehabilitation and patient flow.
* Maintain high quality patient care standards within the ward teams.
* Support the management of three rehabilitation wards (two general and one stroke).
