Thisis an opportunity for a motivated individual to further their career in primaryhealthcare by joining Sacriston Surgery as a Practice Clinical Pharmacist.

You will have experience of working in Primary Care in a similar role, be a qualified and experienced prescriber, and will use your existingskills to support the growing organisational requirements.

Youwill work in collaboration with a range of other teams and stakeholders.Effective communication skills are essential.

Thejob will provide you with a range of different experiences and challenges, workingalongside both clinical and non-clinical staff in a dynamic general practice environment.


Main duties of the job

Working withinyour clinical and professional boundaries as a practice based prescribingpharmacist, within a patient facing role as part of a multidisciplinary team. Thepost holder will comply with the practices policies and procedures.

We are a friendly and welcomingpractice team, with a forward-thinking partnership (4 GPs and 1 non-clinical partner),working from a modern, purpose-built 3-storey eco-friendly building (ample freecar parking) in this former mining village, situated outside of Durham City andeasily accessible to commuter routes (e.g. Newcastle, Sunderland, the A1). Weare an EMIS practice, with a growing patient list of currently circa. 10,700 patients.

We are seekingsomeone who wants to deliver excellent standards of patient care, is forward-thinking, positive, with a 'can do' attitude.We are a training practice, very keen to foster the current and futuregenerations of general practice staff. We offer excellent CPD opportunities, and mentoring for those newlyqualified.

Job responsibilities

The job description is attached for further reference. As a prescribing clinical pharmacist, you will work alongside and be directed and supported by the current practice pharmacist, and will be an integral part of the team, providing a range of services and support to colleagues and patients, working to the following main areas:

Telephone medicines support

Medicine information to practice staff and patients

Management of medicines at discharge fromhospital

Interface & Signposting

Repeat Prescribing

Information management

Implementation of local and nationalguidelines and formulary recommendations

Education and training

Care Quality Commission

Public Health

Person Specification


Personal Qualities & Other

* Self-Motivation
* Adaptable
* Ability and willingness to travel between the practice sites (e.g. for meetings)
* Current Enhanced DBS check
* Immunization status
* Safeguarding adult and children level three


Knowledge and skills

* Able to obtain and analyze complex technical information.
* Recognizes priorities when problem solving and identifies deviations from the normal pattern and can refer to seniors or GPs when appropriate.
* Able to work under pressure and to meet deadlines.
* Gain acceptance for recommendations and influence/motivate/persuade the audience to comply with the recommendations/agreed course of action where there may be significant barriers.
* Work effectively independently and as a team member.
* Demonstrates accountability for delivering professional expertise and direct service provision.
* Experienced EMIS Web user.


Qualifications

* Master's degree in pharmacy (MPharm) (or equivalent).
* Independent prescriber.
* Mandatory registration with General Pharmaceutical Council.
* An appreciation of the nature of PCNs, GPs and general practices.
* CPPE primary care pathway.
* Specialist knowledge acquired through postgraduate diploma level or equivalent training/experience.


Experience

* Minimum of one year's post-qualification experience.
* In depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.
* An appreciation of the nature of PCNs, GPs and general practices.
* An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing.
* Excellent interpersonal, influencing and negotiating skills.
* Excellent written and verbal communication skills.
* Demonstrate the ability to communicate complex and sensitive. information in an understandable form to a variety of audiences (e.g., patients).
* Can plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions.
