Title - Care home falls Practitioner Project Lead
Band - 6 (£38,682 - £46,580)
Hours 37.5 per week
Contract - 12 months fixed term
Responsibilities:
We are delighted to offer a fixed term contract to lead on a project looking at developing falls management within care homes. The successful candidate will work with care homes across Kirklees, through clinical education and coaching support they will look to improve the care homes confidence in the risk assessment and management of the falling resident.
The successful post holder will be registered as an allied health professional or nurse with experience of falls and service improvement. This is a great opportunity to make a difference to those living in care homes, to develop project management skills, and build strong multi agency networks.
The post will sit as part of the Community Rehab Service in the Orthopedic and Frailty Team and will work along side the Care home support team
This is an opportunity for a health professional to steer service development whilst consolidating/ building their own skillset in the area of quality improvement and project management.
This vacancy is advertised on a fixed term contract / secondment for a period of 12 months and is open for internal expression of interest within Locala.
