Job Description Occupational Therapist – Adult Social Care
Working Pattern: Community-based with flexible working options
Local Authority located in Herefordshire is seeking a qualified, HCPC-registered Occupational Therapist to join its Independent Living Service. This role focuses on supporting adults of all ages to maintain independence, wellbeing, and quality of life through strengths-based, person-centred practice within the community.
Main Duties:
Complete statutory Occupational Therapy functional, social, and environmental assessments
Apply a strengths-based approach to identify risks, barriers, and preventative strategies
Support adults in their own homes to engage in meaningful daily activities
Recommend and facilitate provision of equipment, minor adaptations, and specialist community resources
Undertake moving and handling assessments promoting dignity, safety, and independence
Work collaboratively with social workers, health professionals, housing teams, families, and carers
Maintain accurate documentation and work in line with safeguarding and local authority proceduresRequirements:
Degree or Diploma in Occupational Therapy
HCPC Registration
Experience working in Adult Social Care or community settings
Knowledge of relevant legislation and person-centred practice
Strong communication, organisational, and caseload management skills
Eligibility to work in the UK
