Aspects of the role include:
* To assess, plan, implement, promote and evaluate safe, creative and effective treatment plans in collaboration with the patient's diverse needs, their carers and other professionals.
* Deliver interventions that are evidence based.
* Be responsible for the safe monitoring, management and administration of medication.
* Work autonomously, building leadership skills and advanced clinical assessment skills.
* Build effective relationships with patients, their carers, family and colleagues across the MDT.
* Develop exceptional planning and organisational skills.
* Analyse care and treatment provided, and develop sound clinical decision making.
* Be trained to deliver high quality care and develop knowledge in all areas including palliative care, leg ulcer management, acute and chronic wound care, understand bladder and bowel dysfunctions, provide supportive care to carers and family members.
* Delegate and support junior staff, building.
