The role is to support the practice nursing team in the delivery of nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population. The healthcare assistant will deliver and assist clinical staff in the provision of treatment, preventative care, health promotion and patient education. General Practice experience is essential., Our aims are to offer the highest standard of health care and advice to our patients, with the resources available to us. Team structure
* Nursing Team: Registered Nurses and 1 Nurse Associate ensuring continuity of care
* Healthcare Assistants: 2 HCAs supporting patient care and clinical efficiency
* Pharmacy Team: The Team focuses on optimizing medication therapy, working directly with patients and healthcare teams to improve outcomes and safety.
* Admin and Care Navigator Team: The team provides essential support within the GP practice, combining administrative efficiency with patient focused navigation to ensure smooth access to care.
Together, this multidisciplinary team works closely to provide comprehensive healthcare services, with a strong emphasis on teamwork, innovation, and professional development.
