JobDescription:
1. Underthe supervision of the ExecutiveDirector.
2. Provides general medicaldirection and supervision of the medical aspects of participantcare
3. Responsible for the delivery ofparticipant care clinical outcomes and oversight of the QualityAssurance Performance Improvement (QAPI) program.
4. Responsible for the development andmaintenance of medical policies and procedures/standards andprotocols
5. Provides direction and supervision torelated Primary Care Providers (PCPs) andAPPs.
6. Provides leadership and guidance to theprogram colleagues and providers in the localcommunity
7. Need to have a driverslicense proof of insurance and means oftransportation
8. Proficient inMicrosoft Suite (Word Excel PowerPoint etc.) in addition to basicknowledge of email calendaring systems and other software asrequired to perform the essential functions of thejob
9. Excellent organizational customer serviceflexibility and problemsolvingskills
10. Willingness to travel within the regionperiodically as needed
11. Conduct history andphysical on new participants and eventbased and periodicreassessments. Develop medical treatment plan and provide medicallynecessary services in the range of locations in which participantsmay reside.
12. Interview and evaluate applicantsto PACE Organization and their caregivers and participate in theIntake and Assessment decision.
13. Discuss eachparticipants medical conditions and prognosis with him/her andtheir caregiver to assist them in expressing their wishes andplanning their future care.
14. Execute assignmentsin a culture that is shared and collaborative across all divisionswithin TH PACE.
15. Collaborate in the nursingtreatment program of participants.
16. Establishand/or maintain as a member of the leadership team a plan toachieve operating goals.
17. Participate as anactive member of PACE Organization interdisciplinary team. Educatesand advises the team regarding the implications of diagnosescognitive and physical function and treatmentoptions.
18. Participate in team and familyconferences and decisionmakingprocesses.
19. Educate participants and familiesregarding health maintenance and their chronic medical conditionsin cooperation with nursing staff.
20. Supervisethe health care and treatment program as developed by theinterdisciplinary team.
21. Perform physicalexaminations order and evaluate appropriate laboratory anddiagnostic tests and record results in a systematicmanner.
22. Effective skills in physical assessmentand chronic disease management for frail olderadults.
23. Desire to work in a creative alternatehealth care setting.
24. Able to manage changingpriorities according to participantneeds.
25. Participates inprovider oncall rotation asrequired.
26. Minimumof 1 year experience working with the frail or elderly populationis required (no newgrads)
