Esher Green Surgery, a part of Bourne Health, are looking for a proactive and enthusiastic GP to join our Practice, on a six months maternity cover fixed term contract (with a view to extend), to work closely with our nurses and GPs to manage a busy and interesting clinical caseload.
Thepostholder will provide high-quality patient care across both clinical sessionsand telephone triage. They will have the opportunity to contribute to theleadership of key disease areas, monitoring Quality and Outcomes Framework(QOF) trends, supporting staff training and development, and ensuring safe andeffective prescribing. They will also play an active role in developing andimproving healthcare services for a diverse patient population, includingpractice-based commissioning, patient participation and health promotioninitiatives.
Thepostholder will be closely supported by both the clinical and management teamswithin the practice and will be part of a wider multi-site clinical team. Whilethe main base will be at Esher Green Surgery, there may be opportunities todeliver triage and on-site clinical sessions at other Bourne Health GPPractices.

Main duties of the job
In accordance with the practice timetable, asagreed, the post-holder will make themselves available to undertake clinicalsessions. A clinical session is normally composed of face-to-face appointmentstogether with telephone appointments, followed by a variety of duties includingqueries, visiting patients at home, checking and signing repeat prescriptions,actioning test results, medical reports, and examinations (e.g. InsuranceCompanies) on behalf of the Practice. The above duties should be performed in atimely manner. Appointments will be 10 minutes duration.

You will be required to
Act upon most alerts (e.g., QOF) and reminders during a routineconsultation i.e. (10 minutes).
Provide all GP services required as part of the PMS contract heldbetween the practices and the commissioning primary care agency, including allcore services such as Child Health Surveillance
Undertake Duty Doctor and Paperwork Doctor duties on a rota basis.
Undertake surgeries during extended hours in accordance with theaccess needs of the practice population and the contractual obligation of thepractice
Provide cross cover, where necessary and practical, at the otherpractices
Respond to medical problems presented by patients, includinghistory taking, examination, investigation, diagnosis, treatment, and referralwhere appropriate.
Provide appropriate preventative health care and advice.
Provide medical support as required to patients referred by nursingstaff
Provide free certification in line with Schedule 9 of NHS GMSRegulations (1992).
Adhere to any practice prescribing formulary, local and nationalguidelines.
Make professional, autonomous decisions in relation to presentingproblems, whether self-referred or referred from other health care workerswithin the organisation
Record clear and contemporaneous I.T. based consultation notes toagreed standards;
Collect data for audit purposes
Compile and issue computer-generated acute and repeat prescriptions(avoiding hand-written prescriptions whenever possible)
In general, the post-holder will be expected to undertake all thenormal duties and responsibilities associated with a GP working within primarycare.
Other responsibilities with the Practice
Engaging in appropriate lifelong educational and audit activity tomaintain clinical competence and performance, including the completion ofobjectives agreed by you and the practice, in an annual Personal DevelopmentPlan. Your Personal Development Plan must be in line with the PracticeDevelopment Plan.
Actively participating in the training and development of nurses,medical students, and GP trainees in the practice.
Supporting the practice staff and responding to requests for adviceand assistance from the practice reception, secretarial and nursing staff.
Attend prescribing, palliative care, clinical and practicemeetings.
You will be expected to take part in clinical meetings, be preparedto chair these and present clinical topics as required for the maintenance ofeducational standards and high-quality care. You will receive feedback from thepartners via peer review and to act upon recommendations and demonstrate change
Provide additional enhanced services to the practices that meet theneeds of the practices patient population and maximise the income streams tothe practice
Undertake additional training where necessary to provide theseenhanced services
You are required to actively participate in Practice BasedCommissioning - this will include reducing first outpatient attendances, demandmanagement, monitoring of activity including peer review and data validation
Awareness of and compliance with all relevant practicepolicies/guidelines, e.g., prescribing, confidentiality, data protection, health and safety, annual QOF Assessment of thePractice.
Meet all tight timescales/deadlines for audits and written returnsto ensure that the Practice meets quality standards and receives the designatedfunding (e.g., Locally Commissioned Services returns, annual QOF audit etc.)

Post holders must maintain the confidentialityof information about patients, staff, and other health service business inaccordance with the Data Protection Act of 1998. Post holders must not, withoutprior permission, disclose any information regarding patients or staff. If anymember of staff has communicated any such information to an unauthorisedperson, those staff will be liable to disciplinary action up to and includingdismissal. Moreover, the Data Protection Act 1998 also renders an individualliable for prosecution in the event of unauthorised disclosure of information.Following the Freedom of Information Act (FOI) 2005, post holders must applythe organisations FOI procedure if they receive a written request forinformation.
Information Governance
All staff must comply with informationgovernance requirements. RiskManagement All post holders have a responsibility to report risks such asclinical and non-clinical accidents or incidents promptly.
