Dacorum Proactive Care Team - Neighbourhood Link GP
We are recruiting a passionate and proactive GPto join the Dacorum Neighbourhood Proactive Care Service.
This is an exciting opportunity to play a key role ina new team delivering community-based, person-centred care for residents. The focus will be on holistic assessments, care coordination, addressing health inequalities and delivering on the strategic ambition of a 'neighbourhood health service'.
Main duties of the job
You'll work as part of a multidisciplinary team supporting a cohort of residents with moderate to severe frailty who have had 2 or more unplanned admissions within the last year, linking them to health, social care, and voluntary sector services.
This service will see the shift from reactive care to proactive identification by using risk stratification searches within Primary Care records. The searches will identify a cohort of patients who would benefit from targeted intervention to improve outcomes, their overall experience and reduce the likelihood of further unplanned admissions. The service will operate alongside a number of other acute and community services, with the ability to step patients up in to appropriate services - including the highly successful virtual ward models in South and West Hertfordshire.
This role presents as an excellent opportunity to work in a topical area of service delivery within the NHS right now and will provide the opportunity to gain service development skills .

Working alongside nurses, AHPs, pharmacist, and case coordinators, you will be undertaking a range of proactive assessment and interventions to enhance health and wellbeing in this patient group, identifying actions which can be taken to prevent hospital admission, avoid unnecessary admission to care homes, and support patients to live well. This will be delivered in partnership with the patient's usual registered GP.
There is a requirement to have face to face contact with patients in a clinic setting, or in the patients own home including in a care home when appropriate.

Person Specification
Education /Qualification
* Full GMC registration and on National Performers List
* General Practice CCT or equivalent (eg MRCGP, JCPTGP)
* Satisfactory Appraisal within the last 12 months (or within the most recent 12 months of work, if currently on a career break)
* Evidence of continuing professional development
* Higher degree e.g. MSc, MD or other higher degree
* Extended experience in complex case management/geriatric medicine
* training in leadership and/or management
Experience
* Minimum of two years core GMS experience in General Practice
* Primary care home visiting
* Safeguarding training and practical experience
* Face to face consultations
* Demonstrate an ability to lead and motivate individuals and teams
* Ability to anticipate and plan for change
* Good working knowledge of professional requirements and standards for healthcare professionals including appraisals
* Evidence and experience of networking with a wide range of stakeholders including the public, external commercial organisations and national strategic bodies
* Experience in care of the elderly medicine and in A&E/Urgent care setting
* Experience in the audit, quality improvement, clinical governance
* Experience undertaking GP sessions in SW Herts
* Experience in developing patient centred healthcare
