Home First Clinician (Nurse or AHP)
Working across the Surrey Downs system, you will deliver urgent community response and short‑term care to prevent hospital admissions and support safe, timely discharges from acute settings. As part of a 7‑day Home First Service, you will work within the Transfer of Care Hub, rotating across triage, in‑reach and care‑home liaison functions to reduce discharge delays and ensure seamless patient pathways.
As a skilled clinician, you will be responsible for assessment, diagnosis, treatment and evaluation, using sound clinical judgement and decision‑making to manage complex patient needs.
Responsibilities
* Deliver expert, specialist service for adults with complex health and social needs who may benefit from, or who are under the care of, the Home First Service.
* Coordinate a seamless service through the development of enhanced interdisciplinary team processes and communication, within the Home First Service and across the wider Surrey Downs Health and Care system.
* Empower patients and carers to make informed decisions regarding their goals and outcomes and the care they will receive to support them to meet these.
* Support the development and evaluation of clinical protocols and systems of inter‑agency documentation to enhance both continuity and the standards of care.
* Understand and apply the Mental Capacity Act in regard of mental capacity in decision making and appropriate application of Deprivation of Liberty requirements, and work with other agencies within the Safeguarding framework.
* Attend meetings as required, including deputising for the Clinical Manager as requested.
* Develop close links with the wider community to enhance both care for individual patients and the wider development of Epsom Health and Care.
