We are recruiting for a Continuing Care Support Worker who shares our ambition and vision to deliver theBest of Care by the Best Peopleand has the drive to take us forward, by ensuring that the Trust continues to be the first choice for patients and our staff.
Main duties of the job
The successful post holder will be expected to support families in the community who have a child with complex medical needs. This will entail travelling to different sites, so a driving licence is essential.
As a Continuing Care Support Worker you will ensure that a high standard of care is provided by following the child's individual care plan.
In addition, you will be given specific training for the role.

Job responsibilities
Undertake assigned tasks and procedures linked to the Continuing Care Team competencies, Directorate and Trust objectives which are specific to the immediate care environment.
Tracheostomy care and tracheal suction
Changing a tracheostomy tube and tapes/ties
Using respiratory support equipment
Tube and pump feeding
Administration of specific medicines

Qualifications
Knowledge
* Knowledge of the specific needs of children and young people
Experience
* Experience at working with children and young people

* Ability to communicate with the general public
* Ability to work independently
