Our Practice Nurse role would be expected to undertake a range ofnursing assessments and provide appropriate care or treatment in conjunctionwith our clinical team and Practice Policies.
You will required to support and deliver a high level of qualitycare and service based on the needs of the patient population of which wouldinclude treatment, management plans, screening, preventative care and patienteducation. In addition to this, you would also be expected to support thePractice in meeting its contractual obligations and delivery of enhancedservice specifications.

Fleetwood Surgery is an innovative and forward thinking Practice led by our partnership of 3 GPs. We serve and cover a local population of approximately 6,500 patients and are looking to recruit and welcome a Practice Nurse to expand and enhance our team.
We are a friendly, professional and caring team who are passionate about providing safe and effective care. Our excellent multi-skilled Clinical Team comprises of 3 GP Partners, several GP trainees, Pharmacists, Pharmacy technicians, Practice Nurse & HCA's.
Our team also includes a Practice Manager, Assistant Practice Manager, Administrators, Secretaries. Our reception team who work in a Patient Advisory role providing enhanced signposting and ensuring patients have access to the appropriate clinicians or services available to them.
Supported by our excellent Clinical and non-Clinical teams, FleetwoodSurgery is a consistently high performer in relation to its contractualobligations including QOF and Quality Contract services.
An established Training Practice, we pride ourselves in our commitmentto teaching and support of a variety of clinical educational placements.

Job responsibilities
Assess, plan, develop, implement, and evaluate programmes to promotehealth and well-being.
Implement and evaluate individual treatment plans for patients with aknown long-term condition
Identify, and manage as appropriate, treatment plans for patients atrisk of developing long-term condition
Prioritise health problems and intervene appropriately to assist thepatient in complex, urgent or emergency situations, including initiation ofeffective emergency care
Support patients to adopt health promotion strategies that encouragepatients to live healthily, and apply principles of self-care
Deliver opportunistic health promotion using opportunities such as newpatient health checks
Provide information and advice on prescribed or over-the-countermedication on regimes, side effects and interactions, as able to
Assess and care for patients presenting with uncomplicated wounds
Support and advise women requesting information relating to familyplanning needs and general health
Support and manage health needs of women presenting for cervicalcytology consultations
Recognise, assess, and refer patients presenting with mental healthneeds in accordance with the National Framework for Mental Health
Implement and participate in vaccination and immunisation programmes forboth adults and children
Advise, support and, where appropriate, administer vaccinations forpatients travelling abroad
Promote and deliver evidence-based care for patients presenting withaural conditions
Assist senior partners in providing minor-surgery sessions

Person Specification
Qualifications
* Diploma in an area of or in Long Term Chronic Disease Management (Diabetes, COPD, Asthma etc)
* Membership of professional body
* Clinical supervision training and experience
* Independent/supplementary nurse prescribing qualification
Experience
* At least 2 years recent primary care nursing experience within GP practice
* Experience of nurse led management of long-term conditions
* Experience of implementing protocols and clinical guidelines
* Experience of quality initiatives such as clinical benchmarking
* Practice experience of working with others
* Experience of using own initiative
* Knowledge of Primary Care Contract Requirements and application of service delivery to meet targets e.g GMS Contract, QOF, Enhanced Services
* Working as part of a team
