We are looking for anexperienced Occupational Therapist to join our exciting Community Therapy Teamwithin The Rotherham NHS Foundation Trust. This team provides therapy foradults with a very wide range of conditions in the community setting. Adedicated part of the team in-reaches into the Rotherham Hospital EmergencyDepartment, to prevent admissions. The team includes Physiotherapists, OccupationalTherapists, Assistant Practitioners and Therapy Support Workers and worksclosely with a wider MDT of Nurses, Nursing Support Workers, Social Workers, voluntarysector and care staff. The team provides a 7 day service and there willbe an expectation to work weekends, predicted pattern (Tuesday - Saturday). The successful candidate will have experienceworking in a community setting and be used to a holistic approach to assessmentand planning with a very patient centred ethos. Other requirements are goodclinical risk management, equipment assessment and provision, and excellentcommunication skills. We have a strong commitment to supervision, PDR,CPD, student and staff training. The successful applicant will have a full drivinglicense with access to a vehicle for business use and a valid work permit to beeligible to work within the UK.The post is subject to Occupational HealthScreening and Criminal Records Bureau Disclosure checking. Main duties of the job
This role involves workingwithin the Community Therapy Team pathways which includes In-reach to CommunityHospital Admission Avoidance Team, Integrated Urgent Response at Home andIntegrated Rehabilitation & Reablement at Home. There will be anexpectation of flexibility to work throughout the Community Therapy Team. The role will specifically provide the following: Independently provide assessment for patientsunder the care of the Urgent Community Response Pathway. Implement and evaluate therapy interventions forpatients relating to their treatment in line with specific condition pathways. To input accurate patient data onto the ITsystem. To organise and plan own caseload and others tomeet service and patient priorities. Readjusting plans as situationschange/arise in consultation with the team where appropriate. Leadership with delegation, support andsupervision of therapy staff where required induction of new staff/studentmentorship.
