Job Summary:
To work as part of the Planned Patient Care Administration team, to support the clinical teams within the Practice and the Care at Home Team, for all routine and continuity patient needs.
Key Duties/Responsibilities:
* To work closely with all members of the practice team and external organisations to enhance patient care.
* To work closely with the patients of the practice and their family/carers to assist with the smooth running of their care.
* To assist all clinicians with administrative tasks, working within defined protocols. Duties may include;
* Review and action normal test results.
* Review and action letters/correspondence that doctors do not need to see.
* Do the administrative aspect of simple referrals on behalf of doctors/nurses.
* Filter requests for information/advice from patients/carers/family/other health care professionals and signpost to them to the most appropriate person, or service, to deal with the request.
* To screen patient information coming into the practice to alert GP's when an action is needed
* Ensure the booking of appropriate appointments, with the appropriate clinician, for all patients, following the practice protocols.
* Complete care plan templates on the clinical system
* To support the needs of patients recently discharged from hospital.
* Liaise closely with patient and carers on all aspects of care plans.
* Utilise the data and systems available {Urgent Care Dashboard is one example} to alert clinicians when patients may require a care plan review.
* Collect and collate data for audit purposes.
* Attend practice meetings as necessary.
* To deputise for the Non-Clinical Care Coordinator at MDT meetings, as required
Qualifications:
English GCSE (or equivalent) Level C or equivalent experience.
Previous experience that demonstrates good attention to detail.
Previous experience of dealing with the public/patients.
