Promoting independence to avoid the need for citizens to depend on care. Helping the Council keep people at home for as long as possible and keeping costs of care packages down. LIAISON : Liaising with colleagues in the NHS, social services, other Council directorates and voluntary organisations where applicable. A patch system is in operation and effective working relationships with members of the primary health care team and GPs will be expected. Occupational Therapy and other students are an integral part of the team. Fieldwork Education will be undertaken and the post holder may lead or support in this process. MAJOR PURPOSE OF POST To promote and maximise independence, social inclusion and safe care for all clients on caseload. MAIN DUTIES AND RESPONSIBILITIES. To carry out occupational therapy assessments, intervention and evaluation of service provision for adults. (primarily those with complex needs.) To work in the context of an interdisciplinary setting and to liaise with and provide advice to team members and social work colleagues as required To liaise with and work in conjunction with other agencies as required To effectively monitor the progress of recommendations for works of adaptation and equipment. This includes review of provisions as required. To support and supervise the work of junior staff students and Occupational Therapy Assistants as delegated by the Team Manager so as to ensure consistent standards of good practice and activity within the team. To complete administrative tasks relevant to the caseload, in particular ensuring that the computer-based data system is properly populated to evidence activity, assessments and services. Case notes will be contemporaneous with activity To contribute towards the development of services to adults in the Vale of Glamorgan Council catchment area. To prepare for and attend supervision with the team manager or authorised designate.
